Losartan Tablets are used to treat hypertension (high blood pressure) in adults and children over 6 years old. This medication is also prescribed to lower the risk of stroke or heart attacks for patients with heart disease. In diabetic patients with kidney disease, it is used to slow down kidney damage.

Losartan is classified as an angiotensin II receptor antagonist (ARB blocker). It works by relaxing blood vessels within the body, which then decreases blood pressure. This action increases the blood and oxygen supply to the heart.

Angiotensin II receptor antagonists such as Losartan are recommended as a first-line therapy option for adults under 65 years old who are unable to take ACE inhibitors. Losartan is listed on the World Health Organization’s List of Essential Medicines.

Losartan Tablets are produced with a dose of 50mg. These tablets are packaged on blisters and available for distribution as boxes of 30 or 100 tablets.

Losartan is often prescribed along with hydrochlorothiazide to lower the risk of strokes and heart attacks in certain at-risk patients. Hydrochlorothiazide Tablets are also available for distribution through AdvaCare Pharma.

What is Losartan used for?

It is used to treat hypertension. It is also used to help prevent strokes and to treat diabetic nephropathy.

How are Losartan Tablets used?

This medication is intended to be taken orally. The tablets should be taken with a glass of water. They may be taken with or without food.

How long until Losartan starts to work?

It can take up to 6 weeks for the full effect of this medication to work.

Can Losartan be stopped if my blood pressure is normal?

Stopping this medication may cause blood pressure to quickly rise, which can increase the risk of stroke or heart attack. High blood pressure typically has no symptoms, so it is recommended to keep using this medicine unless otherwise directed by a doctor.

Losartan does not cure hypertension, but it can help control the condition.

How should Losartan Tablets be stored?

It is recommended to keep the tablets in their blisters until use and store the medication at room temperature (25°). Though they should be kept out of extreme temperatures, the medicine can withstand brief periods of temperatures from 15-30°C.

What dose should be taken?

Adult Dosing Dosage may vary based on different medical indications:

  • For hypertension, the usual dose is 50-100mg per day, divided and taken as 1 or 2 doses. The starting dose should be 50mg. The maximum dosage is 100mg per day. If the patient is taking a diuretic, their starting dose should be 25mg.
  • For diabetic nephropathy, the usual dose is 100mg taken once per day. The starting dose should be 50mg, and the maximum dosage is 100mg per day. This medicine is intended for patients with hypertension and type 2 diabetes mellitus.
  • For stroke prevention in patients with hypertension and left ventricular hypertrophy (LVH), the usual dose is 50-100mg taken once per day. The starting dose should be 50mg, and the maximum dosage is 100mg per day. Losartan can be given in combination with hydrochlorothiazide. It is important to note that evidence suggests that Black patients may not benefit from this treatment option.
  • For heart failure with reduced ejection fraction (off-label), the usual dose is 50-150mg taken once per day. The starting dose should be 25-50mg, and the maximum dosage is 150mg per day.

Renal Dosing No adjustment is necessary for patients with renal impairment, with the exception of if the patient is volume depleted. Guidelines have not been defined for HD/PD.

Hepatic Dosing For patients with hepatic impairment, the starting dose should begin at 25mg taken once per day.

Pediatric Dosing Recommended dosage for children may vary based on different medical conditions:

  • For hypertension in children over 6 years old, the usual dose is 0.7-1.4mg/kg per dose, taken once each day. The starting dose should be 0.7mg/kg up to 50mg/day. The maximum dosage is 1.4mg/kg per day, up to 100mg per day.
  • For aortic dilation associated with Marfan syndrome (off-label) in children over 1 year old, the usual dose is 1-1.4mg/kg per dose taken 1 time each day. The starting dose should be 0.4mg/kg per dose. The maximum dosage is 100mg per day.

Renal Dosing for pediatrics No adjustment is necessary for children with CrCl > 30. Guidelines have not been defined for children with CrCl < 30 or those on HD/PH.

Hepatic Dosing for pediatrics Guidelines have not been defined for children with hepatic impairment. It is advised to consider the recommendations for adult patients.

What happens if a dose is missed?

The missed dose should be taken as soon as it is remembered. If it is nearly time for the next dose, the missed dose should be skipped. Never take 2 doses at the same time.

Who can use Losartan?

Losartan Tablets can be given to adults and children, but caution is advised for specific groups of patients.

Pregnant The use of this drug should be avoided in pregnant women. If there is an absolute need, it is advised to closely monitor amniotic fluid and the fetus. Based on human data and this drug’s mechanism of action, there is a risk of fetal and neonatal harm, including intrauterine growth restriction, oligohydramnios, renal failure, and death.

Breastfeeding The benefits must be weighed against the risks when considering this medication or women who are breastfeeding. There is no human data available to properly evaluate the risk of harm for the nursing infant. There is inadequate data on the effects of this drug on milk production.

Children Losartan is considered safe to treat hypertension in pediatric patients over 6 years old. Studies have not indicated any problems that are specific to this population. For children under 6 years old, the safety and efficacy have not been established.

Geriatric There are no dose requirements or special precautions needed for the use of Losartan in elderly patients. Data shows that it has a comparative efficacy to other antihypertensive medications for geriatric patients.

Other warnings

Losartan is contraindicated for concurrent use with aliskiren in patients with diabetes mellitus.

For women in their 2nd and 3rd trimester of pregnancy, Losartan and other drugs that act on the renin-angiotensin system decrease renal function in the fetus and increase the risk of fetal or neonatal morbidity and death.

Symptomatic hypotension may occur in patients with volume or salt depletion. It is advised to correct the volume/salt depletion before beginning treatment with Losartan.

For patients with renal artery stenosis, chronic kidney disease, severe congestive heart failure, volume depletion, or other conditions in which renal function partly depends on the renin-angiotensin system, caution should be used as there is an increased risk of acute renal failure. It is advised to monitor renal function and consider stopping treatment if a patient exhibits a significant decrease in renal function.

Side Effects

As with all pharmaceuticals, some unwanted effects can occur from the use of Losartan Tablets.

Common side effects include, but may not be limited to:

  • dizziness
  • back pain
  • cold-like symptoms

Seek medical attention if the following develop:

  • signs of an allergic reaction
  • lightheadedness
  • pain while urinating
  • signs of a kidney problem
  • signs of high potassium levels in the blood

Precautions

Do NOT use Losartan Tablets if:

  • You are allergic or hypersensitive to any of the ingredients.
  • You are in the 2nd or 3rd trimester of pregnancy.
  • You have kidney disease or diabetes and are also being treated with aliskiren.

Due to possible drug interactions, consult with your doctor about any medications, supplements, or herbal products you are taking before your treatment. Losartan is known to interact with many drugs, including diuretics (e.g. spironolactone, triamterene, amiloride), blood pressure medicines, heart medications, lithium, or NSAIDs (e.g. aspirin, ibuprofen, naproxen, celecoxib, diclofenac, indomethacin, meloxicam).

This medication may not be suitable for people with certain conditions, including:

This medication is known to cause harm to a fetus. It is important to contact a doctor if you become pregnant while being treated with this medicine. Do not use this medicine while breastfeeding.

Losartan has not been approved for use by pediatric patients younger than 6 years old.

Potassium supplements and salt substitutes should not be used while taking this medication unless a healthcare professional has indicated otherwise.

Frequent blood tests and blood pressure readings may be necessary during treatment with this medicine.

Losartan may cause dizziness. It is recommended to take care when standing up and to avoid getting up too fast.

It is advised to avoid driving or operating machinery until the effects of this medicine are clear.

Losartan reduces aortic dilatation rate in adults with Marfan syndrome: a randomized controlled trial

Patients diagnosed with Marfan syndrome have an elevated risk of severe aortic complications, often preceded by aortic dilation. This study aims to assess whether treatment with losartan, an angiotensin-II receptor-1 blocker, can mitigate the rate of aortic dilation in individuals with Marfan syndrome.

It is a multicenter, open-label, randomized controlled trial with blinded assessments compared the effects of losartan treatment against no additional intervention in both operated and unoperated adults diagnosed with Marfan syndrome. The primary endpoint was the rate of aortic dilation at any predefined level of the aorta following a 3-year follow-up period, determined through magnetic resonance imaging.

This study included 233 participants (47% female) who were randomized, with 116 receiving losartan and 117 receiving no additional treatment. After 3.1 ± 0.4 years of follow-up, the rate of aortic root dilation was significantly lower in the losartan group compared to controls (0.77 ± 1.36 vs. 1.35 ± 1.55 mm, P = 0.014). Losartan did not significantly reduce the rate of aortic dilation beyond the aortic root trajectory. In patients with a history of aortic root replacement, the rate of aortic arch dilation was significantly lower in the losartan group compared to controls (0.50 ± 1.26 vs. 1.01 ± 1.31 mm, P = 0.033).

The findings indicate that losartan treatment demonstrates efficacy in reducing the rate of aortic root dilation in adults diagnosed with Marfan syndrome.
