Antimalarial drugs target various stages of the plasmodium parasite life cycle or reduce the number of parasites in the blood.

The effectiveness of malaria treatments varies depending on the specific drug and the type of plasmodium causing the infection. However, they have been shown to significantly reduce the risk of contracting malaria and improve survival rates in individuals with severe or complicated cases.

What is the mechanism of common Antimalarial Drugs?

Antimalarial medications work by targeting the plasmodium parasites that cause malaria. They can prevent the parasites from entering and multiplying within red blood cells, or kill the parasites already present in the bloodstream. The exact mechanism of action depends on which antimalarial medicine is used.

Chloroquine and hydroxychloroquine are common examples that inhibit the parasite's ability to break down hemoglobin, while malaria medicines with artemisinin derivatives interfere with the parasite's ability to produce energy.

Types of Antimalarials

Antimalarials are used in the treatment of malaria. Although malaria medicines have one main indication, antimalarials can be divided into a number of drug types.

Aminoquinolines

Aminoquinolines are a group of antimalarial medications that are derived from quinine, a natural substance found in the bark of the Cinchona tree. They work by interfering with the parasite's ability to break down hemoglobin, a protein found in red blood cells, thereby preventing the parasite from obtaining the nutrients it needs to survive. Aminoquinolines, such as chloroquine and hydroxychloroquine, are primarily used to treat uncomplicated malaria caused by Plasmodium species, such as Plasmodium falciparum and Plasmodium vivax.

Diaminopyrimidines

Diaminopyrimidines are a group of antimalarial medicines that target the asexual erythrocytic phase of the malaria parasite. They inhibit the dihydrofolate reductase enzyme, which is essential for the production of nucleic acids, leading to the disruption of the parasite's DNA synthesis, and killing the parasite.

The main indication for diaminopyrimidines is the treatment of malaria caused by Plasmodium falciparum and other species. They are often used in combination with other malaria medicine to prevent the development of drug-resistant strains. Diaminopyrimidines are also used as prophylactic agents to prevent malaria.

Artemisinin

Artemisinin and its derivatives, such as artemether and artesunate, are antimalarial drugs formulated from the plant Artemisia annua. They are used in the treatment of malaria, particularly for uncomplicated cases caused by Plasmodium falciparum. The mechanism of artemisinin and its derivatives involves the generation of reactive oxygen species within the malaria-infected red blood cells. This damages and destroys the parasites, including their ring-stage and mature forms (trophozoites and schizonts).

Artemisinin-based combination therapies, which combine artemisinin or its derivatives with other malaria medications, are commonly used to treat for uncomplicated P. falciparum malaria in many regions where the parasite has developed resistance to other antimalarial drugs. The combination with other antimalarials helps to enhance the effectiveness of treatment, reduce the likelihood of resistance, and ensure complete eradication of the parasite.
