Efficacy of melatonin with behavioural sleep-wake scheduling for delayed sleep-wake phase disorder: A double-blind, randomised clinical trial

Delayed Sleep-Wake Phase Disorder (DSWPD) is marked by difficulty falling asleep at typical times and challenges waking up at necessary times for daytime obligations. This trial evaluated the effectiveness of 0.5mg melatonin, along with behavioral sleep-wake scheduling. This was done to enhance sleep initiation in clinically diagnosed DSWPD patients exhibiting a delayed endogenous melatonin rhythm relative to their desired (or required) bedtime (DBT).

This study included 307 participants, of which 116 received melatonin. Compared to the placebo group, these people had sleep onset 34 minutes earlier. Following melatonin treatment, SE T1 (sleep efficiency at the first attempt to sleep) increased, while PROMIS (Patient Reported Outcomes Measurement Information System) sleep-related impairment, PROMIS sleep disturbance, insomnia severity, and functional disability decreased. Additionally, a higher proportion of patients demonstrated more than minimal clinician-rated improvement after melatonin treatment (52.8%) compared to placebo patients (24.0%) (P < 0.05).

This study shows that administering melatonin 1 hour before desired bedtime (DBT), along with behavioral sleep-wake scheduling, proved effective in improving both objective and subjective measures of sleep disturbances and related impairments in patients with Delayed Sleep-Wake Phase Disorder (DSWPD) who exhibited a delayed circadian phase relative to DBT. The improvements observed were primarily attributed to the sleep-promoting effects of melatonin, complemented by behavioral sleep-wake scheduling strategies.
